Faith Formation Registration

Please visit the Rectory Office during office hours to complete a separate registration form for each child, and to submit the following documents and fee. 
 
Required Documents
  • Child’s Birth Certificate
  • Child’s Baptism Certificate
  • Child’s First Communion Certificate, if applicable
  • Registration fees:
    • One Child: $200
    • $50 for each additional child in the same family
